Conveniently located near the Banner Medical Campus at 9700 N 91st Street Suite A-115 in Scottsdale Arizona. Use N 91st from either E Mountain View Road or E Via Linda to access the location. We are on the east side of the 101 loop. Our offices are located in the Viva Medsuites office on the southeast side of the building. There is a Viva Medsuites sign visible from the parking lot and the entrance is to the right. Handicap parking is available near the entrance. We are located on the first floor.
When you arrive please use the electronic kiosk to check in and have a seat in the patient waiting area. Our medical support staff will escort you back to the doctor when your appointment is schedule.
Important please arrive at least 15 minutes early for your appointment. For example, a 1 PM appointment starts at 12:45 PM.
